Meaning of Ketogenic Diet

A ketogenic diet is an eating pattern that severely restricts carbohydrates and uses fat as the primary energy source to induce nutritional ketosis. It is more restrictive than a general low-carb diet and must be distinguished from diabetic ketoacidosis.

What state does a ketogenic diet create?

When carbohydrates are very low, the liver produces ketone bodies from fatty acids to supply energy to the brain and various tissues. This is called nutritional ketosis. The exact carbohydrate range varies by plan, but generally low-carbohydrate dietthe restrictions are greater.

Even if the fat ratio is high, total energy, protein, vegetables, and electrolyte composition are important. Understanding it simply as a diet of eating lots of butter and meat can lead to nutritional imbalances.

Why does weight change significantly in the early stages?

It differs from nutritional ketosis in terms of concentration and clinical situation. Those with type 1 diabetes and users of certain diabetes medications must be especially aware of the risks.

If you have high blood sugar along with vomiting, abdominal pain, deep and rapid breathing, dehydration, and changes in consciousness, an emergency evaluation is required.

What should be checked before starting?

We check for a history of diabetes, blood pressure medication, kidney, liver, or pancreatic diseases, gallbladder issues, pregnancy, breastfeeding, and eating disorders. Do not reduce or stop medications arbitrarily.

If implementing this long-term, plan for progress monitoring tailored to your current status, including dietary fiber, vitamins, minerals, blood lipids, and kidney function.

How is nutritional ketosis confirmed?

When carbohydrate intake is very low, the liver creates ketone bodies from fat to use as an energy source. Ketones can be confirmed by measuring beta-hydroxybutyrate in the blood, as well as urine and breath, but measured values vary depending on diet, exercise, hydration, and the adaptation stage.

Higher ketone levels do not necessarily mean that fat is decreasing faster or that the diet is healthier. In weight management, total energy, diet quality, sustainability, and test results are considered together.

How does the quality of a ketogenic diet vary?

Even with the same low carbohydrate intake, a diet centered on vegetables, nuts, fish, tofu, and olive oil differs in dietary fiber and fat quality compared to a diet skewed toward processed meats, butter, and cheese. Focusing only on the carbohydrate number may lead to a loss of nutritional balance.

Include non-starchy vegetables, various proteins, and unsaturated fats, and monitor for constipation and micronutrient deficiencies. 'Zero-carb' and 'ketogenic' are not used interchangeably.

Are all adaptation period symptoms normal?

Initial headaches, fatigue, dizziness, and constipation may be related to changes in hydration, electrolytes, and reduced intake, but severe symptoms should not be endured as merely part of the adaptation process. Repeated vomiting, dehydration, severe abdominal pain, rapid and deep breathing, and changes in consciousness require immediate evaluation.

The approach should be re-evaluated if there is a continuous decline in exercise performance, changes in menstruation, obsession with food, or binge eating. Rather than masking symptoms with supplements, first verify the appropriateness of the diet and your health status.

What should be done when stopping or increasing carbohydrates?

When consuming carbohydrates again, glycogen and water are restored, which may cause weight to increase rapidly. Do not mistake this entirely for a regain of fat and return to extreme restriction.

Gradually increase foods containing dietary fiber, such as legumes, whole grains, and fruits, according to individual digestive status, and monitor average weight, blood sugar, and digestive symptoms. If taking diabetes medication, dosages must be adjusted with medical staff for both starting and stopping the diet.

Nutritional ketosis and ketoacidosis are different

Nutritional ketosis in a ketogenic diet is a state where the use of ketone bodies increases due to carbohydrate restriction. Diabetic ketoacidosis is an emergency state that can be accompanied by hyperglycemia and acidosis; they are not the same. In particular, those with type 1 diabetes or users of SGLT2 inhibitors may face risks even with near-normal blood sugar, so self-experimentation with the diet should be avoided.

Urine ketone test strips vary depending on hydration and adaptation status and do not measure the rate of body fat loss. High ketone levels do not automatically guarantee an energy deficit or nutritional balance.

Sustainability and nutritional composition are considered together

Following the initial loss of water, track weekly average weight, waist circumference, lipid tests, bowel movements, and exercise performance. Rather than simply increasing fat intake, consider unsaturated fat sources, sufficient vegetables, protein, and micronutrients.

If you experience persistent vomiting, abdominal pain, rapid and deep breathing, severe thirst/dehydration, or confusion, do not dismiss it as keto flu and seek medical attention immediately. Long-term application requires a professional plan depending on your health status and medications.

Does a darker color in urine ketones mean weight loss is progressing well?

Urine test strips show one type of ketone being excreted, and the color varies depending on hydration levels and the stage of adaptation. A dark color does not directly indicate the rate of body fat reduction or the safety of the diet; results may change even with the same diet as the body's use of ketones changes over time.

Do not further reduce water or carbohydrate intake to increase the color of the test strip. Monitor average weight, waist circumference, dietary composition, bowel movements, and exercise performance. If ketones are detected while you have diabetes or are ill, follow the guidance of medical staff based on blood glucose levels and symptoms.

How should it be viewed if triglycerides decrease but LDL increases?

Following a ketogenic diet, triglycerides, HDL, and LDL may change in different directions, influenced by the types of fats consumed, weight changes, and genetic responses. The improvement of one marker cannot be seen as automatically offsetting the increase of another.

Review the overall lipid results before and after testing, the sources of saturated and unsaturated fats used in the diet, and family history with medical staff. If LDL increases significantly or familial hypercholesterolemia is suspected, do not simply dismiss it as part of the adaptation process; re-evaluate the dietary method and the necessity for treatment.

Is this a high-protein diet?

Does the presence of urine ketones mean fat is being burned well?
