Conclusion of this page

  • Ma Huang herbal medicine, ephedrine-based pharmaceuticals, and overseas ephedra supplements are not the same products, so please distinguish the context in which the data is used.

  • The names Gambijeong or Gambihwan alone do not indicate whether Ma Huang is included or the dosage used, so please verify with your prescribing institution.

  • Hand tremors, throbbing, and insomnia are not evidence of efficacy. If these recur, notify your prescribing institution on the same day before taking additional doses.

Please check your current medications before reading

If you do not know the name, ingredients, and dosage of your medication, you may apply incorrect information even if it is categorized as 'weight loss medicine.' This page does not replace individual prescription changes or emergency medical care.

First, do not confuse these four terms as if they mean the same thing

Ephedrais a plant used as a herbal medicine, called Ephedra or Ma Huang in English. ephedrineis one of several alkaloids that can be found in Ephedra. Purified ephedrine medications and overseas dietary supplements containing ephedra are different products.

The reason this distinction is necessary is simple. Because the overall composition of the product, the actual exposure amount, the accompanying ingredients, and the manufacturing and prescription management differ, the effects and adverse reactions the meaning of the data also change. Neither the conclusion that "since ephedra was banned in the US, all domestic Ephedra prescriptions are the same" nor the conclusion that "since it is a prescription from a Korean medicine doctor, there is no need to look at known risks" is sufficient.

What do overseas regulatory materials tell us, and what can they not tell us?

In the United States, dietary supplements containing ephedrine alkaloids were banned from sale due to concerns over cardiovascular and neurological risks. NCCIH and NIH data state that while short-term weight changes were observed with ephedra/ephedrine supplements, there were accompanying adverse reactions such as nausea, anxiety, mood changes, and pulsating sensations, as well as more serious risk concerns.

This data clearly shows that the expression "natural ingredient" does not exempt one from verifying risksHowever, since the composition, dosage, and combined ingredients of overseas supplements are not the same as prescriptions from domestic Korean medicine doctors, the incidence of harm or product characteristics from those studies cannot be applied directly to my prescription. While the warnings from the evidence should be accepted, the scope of application must be distinguished.

Ephedra cannot be assumed based solely on the names Gambijeong or Gambihwan

Gambijeong and Gambihwan are names that can be used by various medical institutions and products. Even if the names are the same, the inclusion of Ephedra, the combination of medicinal herbs, the formulation, and the daily dosage cannot be assumed to be the same. Do not try to reverse-engineer your prescription based on online reviews or photos of other people's prescriptions.

Verify your medical institution, the exact prescription and medicine name, the main components, whether Ephedra is included, the single and daily dose, and the time of administration. For the actual prescription composition and dosage of Baekrok Gambijeong, the prescription and medication guide issued to the individual take priority, and individual content and eligibility for administration are confirmed during the medical evaluation.

What medical history must be disclosed before a prescription?

highblood pressure, arrhythmia and other cardiovascular diseases, cerebrovascular diseases, hyperthyroidism, glaucoma, anxiety, panic, severe insomnia, and a history of seizures should be reported. The prescriber must also verify whether you are preparing for pregnancy, are pregnant, are breastfeeding, are an adolescent or elderly person, or have a scheduled surgery or endoscopy.

If you know your usual resting pulse·If you know your blood pressure and sleep status, please share it. Do not omit your medical history simply because a reading was normal once, and do not decide on your own that Gambijeong is right for you. History of hand trembling after taking appetite suppressants, cold medications, or herbal medicine·palpitationsor severe headaches after taking appetite suppressants, cold medicines, or herbal medicine in the past, state the medication taken and when it occurred.

Is it an absolute contraindication if there are thyroid or blood pressure issues?

The same answer cannot be given to everyone based on the name of the disease alone. For thyroid disease, the exact diagnosis, current functional status, medications, recent tests, and symptoms such as pulsating sensations, tremors, sweating, and insomnia are considered together. For blood pressure, the usual control status, heart and cerebrovascular history, and medications currently in use are checked, rather than a single numerical value.

The important thing is not to hide the disease and to discuss whether this prescription is appropriate and explore other methods before prescribing. If symptoms are already present or the condition is unstable, evaluation of the existing disease may come before the prescription. If values or symptoms change after the prescription, do not interpret this as a sign of effectiveness, but request an adjustment.

Why are coffee, energy drinks, and cold medicines checked together?

Caffeine can be found in coffee, energy drinks, tea, and high-caffeine supplements. It must be monitored whether pulsating sensations, hand tremors, anxiety, or insomnia are felt more intensely when used with a prescription containing Ephedra. The fact that you "usually drank coffee well" does not guarantee the safety of combined use with a new prescription.

Among cold and rhinitis medications, there are ingredients that can affect heart rate or blood pressure. Please show the medication name and ingredient list to the prescriber or pharmacist. Do not arbitrarily overlap other appetite suppressants, pre-workout supplements, or diet aids, and do not assume that the effect disappears simply by spacing them out by a few hours.

If hand tremors occur, what should be recorded first?

Record the time the tremors started, the time of your last dose and meal, any intake of coffee, energy drinks, or cold medicine, and your sleep duration. If you are taking diabetes medication, check for suspected hypoglycemia symptoms such as cold sweats, severe hunger, or confusion, and record your blood glucose levels if possible.

Hand tremors are not a sign that fat is being burned. If tremors recur while taking Baekrok Gambi-jeong, or if they are accompanied by pulsating, insomnia, or severe anxiety, notify Baekrokdam on the same day before taking another dose. If accompanied by clouded consciousness, convulsions, chest pain, fainting, or difficulty breathing, seek help immediately.

The schedule for dosing, meals, and caffeine, as well as the criteria for contacting us on the same day, can be found Hand Tremor Guide After Prescriptions Containing Ephedrastep-by-step in this guide.

If my heart is pulsating, is it enough to just look at the pulse rate?

If possible, sit and rest for a while before measuring your pulse and blood pressure, and record the difference from your usual levels. However, a single pulse reading within your normal range does not rule out risk. Check whether the beat is regular and if you experience chest pain, shortness of breath, dizziness, or a feeling of fainting.

If pulsating recurs even after resting or interferes with sleep and work, contact Baekrokdam on the same day before your next dose. For chest pain, fainting, severe difficulty breathing, or persistent irregular beats, use 119 or an emergency medical institution without waiting for an online consultation.

Information on measuring pulse and blood pressure and the details to send to Baekrokdam are in the Pulsating and Blood Pressure Guide After Prescriptions Containing Ephedracan be seen continuing in the following section.

At what point should I report insomnia, anxiety, or headaches?

Record the time of dosing, the time you fell asleep, any nighttime awakenings, naps, caffeine intake, and any recent changes in prescriptions. Do not add alcohol or someone else's sleeping pills if you cannot sleep. If you are unable to sleep almost all night or experience severe agitation or panic, notify the prescribing institution on the same day.

Headaches can have various causes, such as dehydration, lack of sleep, or changes in caffeine intake. However, if you experience a sudden and very severe headache different from usual, vision changes, one-sided paralysis, slurred speech, changes in consciousness, or convulsions, you must receive a prompt medical evaluation immediately.

If insomnia or anxiety is the primary concern, Insomnia and Anxiety Guide After Prescriptions Containing Ephedra, or if you have used coffee or cold medicine together, Caffeine and Cold Medicine Co-administration Guideplease check the details for your specific situation in these guides.

Why 'taking more if the effect is insufficient' is dangerous

The amount of ephedra used cannot be determined by a single weight digit or an online recommendation for a 'strong level.' The prescriber must determine the dose and track the response by considering the overall prescription, diagnosis, past reactions, comorbid conditions, concomitant medications, and lifestyle conditions.

Do not arbitrarily increase the frequency or amount even if appetite or weight does not change as expected. Do not assume the dosage is the same when resuming after pausing the treatment for a few days or when switching to a prescription from another institution. Treat it as a new prescription and re-verify the composition, dosing instructions, and follow-up plan.

Safe prescriptions are revealed in the follow-up plan rather than the start date

You should be treated at a medical institution with a clear source, and you must be provided with the actual prescription, dosing instructions, and a channel for inquiries. Before the first dose, record your usual pulse, blood pressure, and sleep; during follow-up consultations, evaluate not only weight but also hand tremors, pulsating, insomnia, anxiety, diet, and bowel movements.

Please determine the following three things in advance.

  1. Which symptoms require contacting the clinic on the same day before taking additional doses
  2. Which emergency symptoms require going immediately to 119 or a medical institution
  3. When to evaluate both effectiveness and discomfort to discuss adjustment, change, or discontinuation

Whether a medication can be taken cannot be determined solely by the fact that it contains Ephedra. Who received which prescription, what was verified before taking it, and what changes were monitored after taking itare all verified during the medical evaluation.

How does Baekrokdam handle questions regarding Ephedra during medical evaluations?

At Baekrokdam, we do not simply end the conversation with a yes or no answer to “Does this contain Ephedra?” Korean medicine doctor Choi Yeon-seung verifies cardiovascular and thyroid history, usual pulse and sleep patterns, current medications and caffeine intake, and previous treatment responses to determine if herbal medicine in tablet form is appropriate and to decide together what needs to be monitored.

Baekrok Gambi-jeong is one of the prescriptions that can be selected after this evaluation; its ingredients and dosage should not be compared to other Gambi-jeong or Gambi-hwan based on the name alone. If you are experiencing symptoms while currently taking the medication, determining the urgency of the situation takes priority over a prescription consultation. If there are no emergency symptoms, please contact us with the medication you took, the medication guide, and a timeline of your symptoms.

Verification of Evidence

What the evidence says and what it cannot say

Data on overseas Ephedra and Ephedrine supplements have confirmed concerns regarding cardiovascular and neurological risks along with short-term weight loss effects.

Meaning in My Situation
The fact that a product contains 'natural ingredients' must not be a reason to skip verifying pulsating sensations, blood pressure, neurological signals, and combined ingredients.
Limitations of Interpretation
Overseas supplements differ from prescriptions by Korean medicine doctors in terms of composition, dosage, quality control, and regulatory systems, so the risk rates cannot be applied directly.

Domestic safety guidelines for the use of Ephedra cover target subjects, risk factors, and the observation of adverse reactions during use.

Meaning in My Situation
After confirming whether Ephedra is included, one's cardiovascular, thyroid, and sleep status must be reviewed along with other stimulant ingredients.
Limitations of Interpretation
General guidelines do not replace the determination of the quality of medicinal herbs, the overall composition of an actual prescription, or an individual's appropriate dosage.
Guidelines for the Safe Use of Ephedra in Obesity Prescriptions ↗

Domestic clinical guidelines for weight loss herbal medicine explain Ephedra prescriptions within the framework of individual diagnosis, safety evaluation, and follow-up management.

Meaning in My Situation
Pre-prescription interviews and the tracking of pulse, sleep, and symptoms after administration are more important for actual safety management than reviews or online dosage charts.
Limitations of Interpretation
Since the prescriptions and subjects in the included studies vary, they cannot be used as data to guarantee the efficacy or safety of a specific product.

Questions

Frequently Asked Questions

Are Ephedra and Ephedrine the same thing?

Ephedra is a botanical medicinal herb, and Ephedrine is one of several alkaloids that can be contained within Ephedra. A prescription containing Ephedra should not be viewed as having the same dosage concept as a purified Ephedrine pharmaceutical.

How much Ephedra is contained in Gambi-hwan or Gambi-jeong?

This cannot be known by the name alone. Since the composition may vary by institution, product, and individual prescription, please check your own prescription, medication guide, and the explanation provided by the prescribing institution.

Can I drink coffee while taking herbal medicine containing Mahuang?

Caffeine may increase feelings of throbbing, hand tremors, and insomnia. Avoid combining them during the initial phase or dosage adjustment period, or when symptoms occur. Please inform your prescriber of your usual caffeine intake to receive specific guidance.

If my hands shake after taking Mahuang, can I just wait until I adapt to it?

Hand tremors are not an indicator of efficacy. If they recur or are accompanied by throbbing or insomnia, notify the prescribing institution on the same day before taking further doses. If you experience chest pain, fainting, or difficulty breathing, seek medical evaluation immediately.

Is it safe to take Mahuang if my blood pressure is normal?

Safety cannot be confirmed by a single normal blood pressure reading. Your usual readings, pulse, cardiovascular and thyroid history, other medications and caffeine intake, as well as the actual prescription and your reaction to it, must be evaluated together.
