Edema is a condition where fluid accumulates in the tissues, causing part of the body or the entire body to swell. There are temporary causes such as salt intake, posture, and the menstrual cycle, but heart, kidney, or liver diseases, blood clots, and medications can also be causes. Therefore, sudden swelling on one side or difficulty breathing should not be dismissed as simple weight fluctuation.

What is edema?

Edema is a condition where fluid accumulates between tissues due to an imbalance in the movement and drainage of fluids inside and outside the blood vessels. It can appear in areas affected by gravity, such as the ankles and calves, as well as in the hands, face, or throughout the entire body. It is a name for a symptom, not a single cause.

It can occur temporarily after standing for a long time or eating salty meals, but heart, kidney, or liver diseases, venous or lymphatic issues, and medications can also be causes. The location and speed of the swelling, along with accompanying symptoms, must be evaluated together.

How is it different from an increase in body fat?

Body fat generally changes over several days to several weeks or more energy balancewhereas edema can change rapidly within a few hours to a few days due to fluid movement. Clues include shoes or rings suddenly feeling tight or an indentation remaining when the skin is pressed.

Is it the same as water weight?

Water weightWater weight is a broad term describing the portion of the number on the scale attributed to fluid changes. Edema refers specifically to the clinical state where fluid is abnormally accumulated in the tissues. Everyday expressions like 'watery flesh' do not reveal the cause. Edema is not diagnosed solely based on body composition analyzer results showing high body water.

What are common temporary causes?

Sitting or standing for long periods, hot weather, and higher than usual sodium intake, long-distance travel, and menstrual cycles can affect mild swelling. Inflammatory responses after exercise and water retention due to carbohydrate intake can also cause weight fluctuations.

Mild swelling in both ankles in the evening that improves overnight should be handled differently than sudden, painful swelling in one leg. Do not group these together as simply 'due to dieting.'

What are the disease-related causes?

Heart failure can cause venous pressure and fluid retention, while kidney disease can lead to problems with sodium and water excretion. Liver disease and severe protein deficiency affect blood proteins and fluid balance. Decreased venous function or lymphatic flow disorders can cause chronic swelling in specific areas.

Thyroid disease, infections, and inflammation can also be causes. Since the treatments differ, do not attempt to diagnose the cause solely through diuretic teas or a low-sodium diet.

Can medication cause swelling?

Some blood pressuremedications, such as anti-inflammatory painkillers, steroids, hormone medications, diabetes and other treatments may be related to edema. If symptoms appear after starting a medication or changing the dosage, record the medication name, start date, and the location of the swelling.

Even if medication is a possibility, do not stop taking prescribed medicine arbitrarily. Sudden swelling of the face, lips, or tongue and difficulty breathing may indicate allergic angioedema and require immediate assistance.

How should I check for indentation when pressing?

Pitting edema is when an indentation remains after pressing the shin or ankle for a few seconds. This can be a clue that fluid has accumulated in the tissues, but you cannot determine the cause by grading the depth of the indentation at home. Some types, such as lymphedema, may be firm and not leave an indentation easily.

Do not press repeatedly or forcefully, and avoid irritating areas where the skin is thin. If there is redness, heat, pain, or wounds, take a photo, record the time of occurrence, and seek a professional evaluation.

How should I record my weight?

If possible, weigh yourself in the morning after urinating while wearing similar clothing, and record any swelling in the ankles, hands, or face, as well as changes in shortness of breath and urine volume. A rapid increase over 2 to 3 days combined with symptoms can be more important than a single day's number.

Those who have received specific weight alert criteria due to heart or kidney disease should prioritize that plan. Do not replace your prescribed guidelines with general internet standards.

How should I manage sodium and water?

Since salty foods can affect fluid retention, it is helpful to monitor the intake of soups, processed foods, and sauces. However, extremely reducing salt and water without knowing the cause can lead to hyponatremia, dehydration, and dizziness.

Sodium and water targets for heart failure and kidney disease vary depending on the stage of the disease and the medication. If you have been given criteria by a medical professional, check the nutrition labels on packaging and your actual intake according to those criteria.

How should I handle swelling during a diet?

If weight increases after eating out, menstrual cycles, or changes in exercise, return to your usual plan and observe the trend for a few days rather than skipping meals immediately. Light walking, changing posture, and elevating the legs can help with some mild swelling, but do not massage a single leg that is painful.

Using saunas, diuretics, or laxatives to lower the number on the scale is not body fat reduction and carries a risk of electrolyte imbalance. First, distinguish the cause and the level of urgency.

Why is it important if only one leg swells?

If one calf or leg suddenly swells and becomes painful, hot, or changes color, you must check for causes including deep vein thrombosis. Please note if you have recently had long-distance travel, were unable to move for a long time after surgery, or are pregnant orpostpartumawareness, and whether you are using hormone medications.

If shortness of breath, chest pain, coughing up blood, or fainting occur, it may be an emergency where a blood clot has moved to the lungs; seek emergency help immediately.

What tests are performed?

During the medical evaluation, the onset and pattern of symptoms and current medications are checked, and heart and lung sounds, as well as the condition of the skin and veins, are examined. Depending on the need, blood and urine tests, ECG and chest exams, venous ultrasound, or heart, liver, and kidney evaluations are selected.

The same tests are not performed for all types of swelling. The body water ratio from a body composition analyzer is not a substitute test for identifying the causative disease.

When to seek immediate help

Prompt medical evaluation is necessary for shortness of breath, chest pain, fainting, sudden swelling and pain in one leg, swelling of the face, lips, or tongue, or systemic swelling accompanied by a significant decrease in urine output. Sudden swelling of the face and hands, severe headache, or visual disturbances during pregnancy also require immediate obstetric evaluation.
